Unable to add data to the data table & query that data using a particular entity.

I was trying to add data to the data table service(that is inside kore ai platform) through invoking digital form by initiating dialog task from the user utterances.
getting the below error while adding data.

For getting data also, the number of rows returned it is showing as zero as below error snap 2.
Any suggestions from any one will helpful…Thanks

Can you please share a little more information on how exactly you are using the entity with screenshots? Also please share the complete curl ( obfuscate the jwt)
